Fitness to Work Certificates (Sick Notes)
If you are unable to work because of illness, you may need to provide your employer with evidence of your absence. This used to be known as a sick note or sickness certificate, but it is now called a fit note (Statement of Fitness for Work).
In many cases, you do not need to contact your GP straight away. You can usually self-certify your illness for the first 7 calendar days of your absence from work. This includes weekends and bank holidays.
Self-certifying your illness
If you are off work for 7 days or fewer, you should tell your employer that you are unwell and complete a self-certification form if they require one. You do not need to see a GP or request a fit note during this time.
Your employer may provide a self-certification form for you to complete. If they do not have their own form, you can download an Employee's Statement of Sickness (SC2) form from the GOV.UK website.
When do I need a fit note?
If you are unable to work because of illness for more than 7 calendar days, you may need a fit note from a healthcare professional. This confirms that your health condition affects your fitness for work.
You should request a fit note through our practice using eConsult. Please provide details of your condition, how it affects your ability to work, when your symptoms started, and any relevant information about your job.
A GP or another appropriate healthcare professional will review your request and decide whether a fit note is needed. You do not always need an appointment, and your request may be dealt with through an online assessment, telephone consultation, or another suitable method.

Extending a fit note
If you are still unwell when your fit note is due to expire, you can request an extension through eConsult. Please submit your request before your current fit note ends where possible.
When requesting an extension, include details about whether your condition has improved, stayed the same or worsened, and how it continues to affect your ability to work.
Fit notes and returning to work
A fit note does not always mean that you must stay away from work. Your healthcare professional may suggest ways your employer could support your return, such as amended duties, a phased return, altered hours or workplace adjustments.
Your employer will discuss any recommendations with you and decide what support can be provided. A fit note is intended to help you and your employer have a conversation about your health and your ability to work.
